The usage of modifiers is a necessary part of the coding process. Therefore, it is important that you know how to use all modifiers according to the chosen evaluation management (E/M) or procedure code. As stated in the Medscape article called “The World of Modifiers:” Modifier 25: Significant, Separately Identifiable E/M Service by the Same Physician on the Same Day of the Procedure or Other Service 🔸Use to indicate that on the same day as a procedure, the physician performed an E/M service that is a significant, separate, identifiable service from the procedure. 🔸Append modifier 25 to the E/M code on the same day as a minor procedure (0 or 10 global days per Medicare). 🔸Link diagnosis code(s) appropriately to E/M and the procedure. 🔸If a patient presents for a planned procedure based on a prior encounter, bill only for the procedure. CMS requires a non-covered form to be completed as a protection for their beneficiaries. But, the form also benefits you to ensure you are paid for services that Medicare does not cover. As shared on our 3/25/2019 previous “Tip of the Day,” per the Infection Control & Hospital Epidemiology survey, 39% of workers made errors removing PPE, including gowns and gloves, increasing the incidence of contamination. Tip: Reevaluate the payment experience for patients. Recently, InstaMed released the ‘Trends in Healthcare Payments Ninth Annual Report: 2018.’ They state: This year’s report is designed to examine the most impactful healthcare payment trends from the unique perspectives of the key industry stakeholders: consumers, providers, and payers. Here are a few of their findings: 🔹70% of consumers are confused by medical bills. 🔹56% of consumers would not be able to pay a $1,000+ medical bill. 🔹93% of consumers were surprised by a medical bill in 2018. 🔹77% of providers say that it takes more than a month to collect any payment. 🔹71% are confused by Explanation of Benefits (EOBs). First, we must stop, listen, and shift our mindset. When you review these results, the keywords that stick out to us are: “confused,” “surprised,” “can’t pay,” “want a payment plan.” Patients are directly telling us that they need help with navigating the payment system so that they can pay you on time. Therefore, it’s time for healthcare entities to stop doing what they think is working and revamp their payment systems in 2019….. Friends, I talk a lot about learning from life and compliance mistakes. But, I am also a firm believer in reflection. I always like to remind myself of the 4 “Its,” in which “It” stands for the actual mistake that I am trying to evaluate. Tip: Review your insurance contracts for information about professional courtesy and waivers of payments.  And, consider creating a financial hardship policy. A common compliance concern is waiving copays/deductibles or providing discounts. Remember, when practices continuously waive patient financial responsibilities it affects the negotiated insurance contract.  It is required that both parties adhere to what is stated in the agreement, which is to collect the owed amount from the patient.
